I distract Ariel with a few hard swats from a thick rubber paddle, and then I distract her again when I make her gag on the handle of the paddle. I clamp her nipples before using some small medical forceps as a septup clamp and nose hooks at the same time. As you can probably tell by Ariel's reactions, this is quite painful...
